Jeff Smith, CDT, offers a step-by-step look at how to fabricate esthetic, monolithic restorations in a single press sequence with subsequent glazing using Ivoclar Vivadent’s new IPS e.max Press Multi.
After meeting at LAB DAY West 2013, Sully Samartzis invited LMT’s Judy Fishman to visit his laboratory when she arrived in Arizona later that year. The laboratory is a pristine addition to a gorgeously appointed home, designed by wife Debbie, a professional interior decorator.
In 1999, Sully Samartzis had a toothache on #13. Treatment meant a root canal procedure followed by a crown. Of Greek descent, he’d only been in the U.S. for three months, arriving here from Germany with a degree in electronics and $1,400 to his name.

Once she arrived in Arizona last fall, LMT Publisher Judy Fishman visited Scott Atkin, Owner of Creative Milling Dental Laboratory and CEO of Dental Lab Milling Supplies, LLC, in Scottdale, AZ, just as Bentley, Atkin’s massive Bernese Mountain Dog, was headed out for a walk with one of the staff members. Atkin “struck gold” in 2007 by adding a retail component to his business model to stay ahead of what’s trending and increase his revenue base.
“The focus for labs these days needs to be on creating win-win opportunities for themselves and their clients,” says...

LMT’s 2014 Fee Survey respondents offer a mixed picture of their sales and profitability. On one hand, laboratory sales seem to be slowly improving. Profits, though, seem to be lagging behind.

The challenging aspect of this case was that one of the implants was improperly placed between #7 and #8. Steve Lee explains how he made a screw-retained zirconia framework with pink gingiva tissue to recreate the lost gingiva and then cemented on four single crowns made from IPS e.max to the framework.
In order to provide patients with a good color match for their restorations, I’ve found I need to do more than work with traditional shade tabs and photos. Here is my step-by-step technique for a 60-year-old patient who needed endodontic treatment and crowns on #9 and #10.

With collaborative partnership and strong leadership, we can connect to the heartbeat of innovation and see improved efficiencies and growth throughout our profession.”
Highlighting the event was the worldwide launch of the new Wieland Zenotec®...
Custom Automated Prosthetics (CAP) introduced Multi FZ, a multi-layered zirconia that makes it possible to achieve an esthetic, multi-colored, monolithic, high-strength (1,100MPa flexural strength), milled full contour zirconia restoration without dipping or green-state shading. The 98-mm discs mimic natural dentition with cervical, body and enamel shading and a “blend zone” at each shade interface. For four- or five-axis milling machines, the discs are competitively priced and come in three different colors and white in order to match 16 VITA shades and bleach shades. CAP Multi...

The company also introduced Ceramill IOS, a new process that integrates Ceramill design software and milling units with third-party intraoral scanners. A new partnership between Amann Girrbach...
Dentorium’s Ariel Marero and Lenny Heller discuss the company’s new Monarch three-layer VITA shade teeth with Steve Cantor, CDT, of Suburban Dental Lab in Bloomfield, CT. The teeth offer multi-cross-linked technology to ensure the look and feel of natural dentition. They offer abrasion resistance, hardness, enhanced toughness, reduced chipping and improved color stability, and come with a five-year guarantee against excessive wear. For a sample, call 800-847-4209 or 212-691-6464.
B&D introduced ORIGIN® LIVETM Multi Zirconia, pre-shaded, multi-layered discs; each layer mimics the inherent gradations of natural teeth, including chroma, value, incisal effects and translucency. Since there’s no dipping or drying involved, the pre-shaded disks streamline production and are less technique sensitive than the coloring liquid method, according to B&D.
The company also demonstrated its new ORIGIN-Haas 5HD milling machine with improved milling strategies (ORIGIN CAM Plus+ powered by hyperDENT) for fabricating custom abutments, implant bars and implant...
For years, porcelain was the material of choice for denture teeth because of its ability to replicate the appearance of natural dentition. But as we entered the 1990s, acrylic denture teeth had replaced porcelain as the industry standard; in fact, the use of porcelain teeth had dropped 50% during the previous decade.
Acrylic teeth offered several functional advantages: they were kinder to opposing dentition with less trauma to the bone and offered easier occlusal adjustment. However, earlier materials tended to craze and check and weren’t as esthetic as the tried-and-true porcelain. Over...

DENTAL TEAM: Scott Atkin, Creative Dental Laboratory and Dr. Michael Foley, both in Scottsdale, AZ
RESTORATIVE PROBLEM: A 79-year-old male patient with an existing PFM full mouth rehab needed a classic A3 shade replacement bridge for teeth #11 through #15.
TREATMENT PLAN: A five-unit Crystal Diamond full contour bridge was prescribed. Crystal Diamond was chosen for its excellent translucency and strength, ideal for highly esthetic anterior and posterior metal-free restorations.
FABRICATION PROCESS: The case was scanned using a 3Shape D710 scanner and milled using Delcam software on a DentMill...
DENTAL TEAM: Ken Upright, Oxford Crown & Bridge, LLC and Dr. Shelley Olson, DDS, FICD, Granville Family Dentistry, both in Oxford, NC
RESTORATIVE PROBLEM: A female patient fractured the entire facial surface and buccal cusp of tooth #4, which had an old MOD amalgam restoration. Due to a 2mm diastema, the entire mesial aspect of the crown would be exposed in the patient's smile, so shade quality was a foremost concern.
TREATMENT PLAN: Because of her positive experiences with the material, Dr. Olson prescribed a Delta Zirconia full contour crown to restore tooth #4. High Translucent Delta Zirconia,...
DENTAL TEAM: Gyu Cho, CDT, 3D BioCAD Milling Center, Renton, WA and Dr. Ronald Bryant, Seattle
RESTORATIVE PROBLEM: The patient presented with severe occlusal wear on his natural teeth; the lower anterior teeth were also very crooked and discolored. The existing crowns also exhibited wear, tissue retraction, dark margins and uneven shades from one crown to the next. Teeth #5, #12 and #14 were missing, requiring implants.
TREATMENT PLAN: A full-mouth restoration with 28 units was proposed and had full compliance from the patient. The team opted to use gold-hue abutments to provide a very natural,...
DENTAL TEAM: Cindy Vuong and Dr. Michael DiTolla, both of Glidewell Laboratories, Newport Beach, CA
RESTORATIVE PROBLEM: The patient presented with a pre-existing discolored PFM restoration on tooth #8.
TREATMENT PLAN: Dr. DiTolla prescribed a full contour BruxZir® crown, and decided to take the opportunity to test the translucency of Glidewell Laboratories' new BruxZir™ Shaded material, which offers complete color penetration all the way through the restoration for greater shade consistency. The new material also prevents shade changes or white "show-through" after occlusal adjustment....
DENTAL TEAM: Scott Gallacher, Precision Milling Center, West Valley City, UT, and Steven Barker, DDS, Salt Lake City, UT
RESTORATIVE PROBLEM: The patient was missing a second bicuspid (#12), didn't want an implant crown and was also concerned about the reduction of the adjacent healthy teeth.
TREATMENT PLAN: To address the patient's desire for the most minimally invasive solution, the team opted for a full contour bridge with minimal retention, using ORIGIN LIVE Plus zirconia since the case required both high strength (for minimal enamel removal) and high translucency (for full contour). Dr. Barker...
DENTAL TEAM: Sven Jesse, Jesse & Frichtel Dental Labs and Dr. Edward Narcisi, DMD, both of Pittsburgh, PA
RESTORATIVE PROBLEM: The patient—the wife of one of the laboratory owners--had an existing all-ceramic restoration on tooth #30 that cracked (she had previously had a PFM crown that also chipped).
TREATMENT PLAN: Since functionality and esthetics were a priority, a Zirlux FC2 full contour zirconia crown was chosen. The laboratory prefers to use Zirlux FC2 because of its flexural strength and low wear against opposing dentition. The Zirlux FC2 system consists of five pre-shaded...
