RESTORATIVE PROBLEM: The patient was missing a second bicuspid (#12), didn't want an implant crown and was also concerned about the reduction of the adjacent healthy teeth.
TREATMENT PLAN: To address the patient's desire for the most minimally invasive solution, the team opted for a full contour bridge with minimal retention, using ORIGIN LIVE Plus zirconia since the case required both high strength (for minimal enamel removal) and high translucency (for full contour). Dr. Barker prescribed a DigitalPrep™ WellBeing™ bridge, a system that allows him to create a digital prep using a 3D image of the pre-op model; the restoration is fabricated before the tooth is actually prepped in the patient's mouth.
FABRICATION PROCESS: Using DigitalPrep software, the distal area of tooth #11 was minimally reduced in the digital file and a retention groove was created. The mesial, buccal and lingual sides of tooth #13 were also minimally reduced, preserving the original occlusion.
The full contour restoration was designed using the ORIGIN Intelligence S™ scanner and milled on the ORIGIN 40 milling machine at Precision Milling Center.
Next, Gallacher first applied B&D's Incisal Enhancer™ on the incisal third for a lifelike effect. The restoration was then dipped in the ORIGIN CHROMA™ liquid (version 3.0) for one minute to match the prescribed tooth shade 2M2; ORIGIN CHROMA liquids were preferred over shaded discs because it was easier to accurately reproduce the prescribed color. Since 2M2 is lighter in brown hue, the VITA® Master 29 coloring liquid was used instead of the Classic 16 shading system.
The bridge was sintered in the DuoTron oven in the fast mode for two hours. Per the instructions for use, no vacuum was applied while glazing in order to avoid potential inconsistencies or chroma change.
The PrepMatrix™, a preparation guide based on the DigitalPrep software, was 3D printed using the Objet Eden 260V and delivered to the dentist with the final restoration. Using the PrepMatrix, the preparation and restoration seating were completed in the same visit in about 20 minutes.
